Written consent must be provided from parents via the school office before any medicines can be administered to a child at school. Please collect a consent form from the school office or download and print one via the link below. Medicines will only be accepted for administration if they are:

· Prescribed for more than 3 x per day

· In date

· Labelled

The medicine must be provided in the original container as dispensed by a pharmacist and include instructions for administration, dosage and storage. The exception to this is insulin which must be in date but will generally be available inside an insulin pen or pump, rather than in its original container. If your child requires an asthma inhaler, these should be brought to the school office clearly marked with the child’s name and class.
